There is a database managing forms (Phil-a-Form) and an e-commerce module (VirtueMart) to process orders via PayPal. This is integrated with a community management component - we use the very popular Community Builder for our user management.
So how do we do all this? It is based on the Joomla content management system. This is free, open-source software available at www.joomla.org. This CMS has been availble for more than 8 years now and we have been working with it for three years. All of the sites you see in the banners above (think advertising) are using Joomla.
